YSR Navasakam Application Form 2021 Online
Volunteers will divide and survey door to door in all villages and mandals of the State. Volunteers will inform House members before their visit. They will take details weather they are eligible to 5 cards i.e. Rice Card, Arogyasree Card, Pension Kanuka Card, Jagananna Vidya Deevena Card, Ammavodi Card. After taking details they hand overs that information to Village and ward Scretariat Functionaries on day to day basis.

Ap has 13 Districts and YSR Navasakam Application Form In Telugu is Available in District wise for Anantapur, Kurnoot, Prakasam, Guntur, Cuddapah, Chittoor, Krishna, West Godavari, East Godavari, Visakhapatnam, Vizianagaram, Srikakulam.
At first AP Government thought to consider beneficiary details from Smart Pulse Survey but later changed decision and decided to start new Navasakam Survey.
A total of about 16000 village and ward volunteers are Participating in this Door to Door Survey.
